Complete the application
List all household members on the application
Sign and date the application
Provide all household income verification for 3 months
(2 months for renewals)
Provide Citizenship and Identity verification (New)

If self-employed, provide your most recent tax return, including all schedules and attachments if filed. If a tax return is not available you must provide 3 months of business ledgers.

Household income verification is needed to complete the application and renewal process. All applications will be screened for HealthWave 19 (Medicaid) first. Children in households with income up to 200% of the Federal Poverty Level may qualify for HealthWave. Below you will find the 2008 Federal Poverty Level Table. If your family income is close to or below these guidelines we encourage you to apply.
